Medicaid Homecare

          Long Term Care manages the Personal Care Program for Medicaid recipients with a medical disability, a physician’s order is required.

          The Medicaid personal care program provides case management, personal care, meal prep, housekeeping, laundry and shopping services when a client is assessed to need those services.

          CDPAP(Consumer Directed Personal Assistance Program) is a personal care program which allows the medicaid recipient to hire and manage their own care provider. Approval for this program is at the discretion of the RN case manager. This program is administered by the Finger Lakes Independence Center - 272-2433.

          PDN(Private Duty Nursing) Long Term Care Services assesses for and prior approves hourly LPN and RN services in the home. Physician's order required.

          LTHHCP(Long Term Home Health Care Program) is a program for nursing home eligible medicaid recipients who may need services not available in the home via the traditional personal care program. This could include in-home respite, medical social worker, and nutritional counseling. Tompkins County Long Term Care Services and The Visiting Nurses Service of Tompkins County and Ithaca administers this program - 273-0466.

          PERS(Personal Emergency Response System) - Long Term Care administers the Medicaid PERS Program, we contract with two area vendors for this service. PERS is a low cost service that can improve the recipient's safety when home alone. It is a communication system that links an individual with the county's emergency medical response system. Pressing a button on the unit, pendant or wristband automatically dials an emergency rescue dispatcher. For more information, contact Long Term Care Services.
